Activating the ELIQUIS Co-pay Card

The ELIQUIS Co-pay Card can help eligible patients save on their out-of-pocket costs for ELIQUIS.

Here’s how to activate it and answers to some frequently asked questions:

You can activate your ELIQUIS Co-pay Card online or by phone:

Online: Go to [invalid URL removed] and click on “Activate Co-pay Card”. Follow the on-screen instructions to activate your card.
By Phone: Call 1-855-ELIQUIS (354-7847) to activate your card with the help of a representative.

- You must be 18 years of age or older and a resident of the US or Puerto Rico.
- You must have commercial insurance that covers ELIQUIS but requires a co-pay.
- You cannot have prescription coverage through a state or federal healthcare program like Medicare, Medicaid, or VA.

- You may pay as little as $10 for a 30-day supply of ELIQUIS (up to 74 tablets for the first fill, then up to 60 tablets for refills).
- The card provides benefits for up to 24 months with a maximum annual benefit of $6,400.

- The card is accepted at participating pharmacies. You can find a participating pharmacy on the ELIQUIS website or by asking your pharmacist.

You may be eligible for the Free 30-Day Trial Offer for ELIQUIS^® (apixaban) if:

1) You have not previously filled a prescription for ELIQUIS.

2) You have a valid 30-day prescription for ELIQUIS

3) You are being treated with ELIQUIS for an FDA-approved indication that an HCP has planned for more than 35 days of treatment;

4) You are 18 years of age, or older; and

5) You are a resident of the United States or Puerto Rico
